How do we help?
We will first assess the individual or families situation against a set criteria to see if they are eligible for support & to understand their needs. We then provide direct support in the purchase of items or the giving of gift cards. We cannot give cash payments.
Process to follow:
1. Nominate a family or individual by completing this form
2. SRM team will assess the situation against a set criteria
3. Person who has nominated the individual or family is notified of the outcome
4. If successful, SRM will directly contact the individual or family to provide the direct support agreed upon.
