In SMS activities and gatherings young persons may reach out to the Priest and leadership team regarding challenges they are facing and support they might need. By agreeing to this consent form, you give permission for your child to speak to the priest in person or via telephone and text messages.

I grant permission to capture, store, and publish photographs and video recordings of my child on official social media channels, websites, and promotional materials. I agree that these materials will be used solely for organisational promotion, that no financial compensation will be provided, and that my child's full name or private details will not be disclosed to protect their privacy in accordance with NSW Child Safe Standards. 

All information collected will be kept confidential and private in accordance with the Privacy Act 1988 (Cth) and relevant professional obligations. The SMS leadership team will access collected information for the purpose of catering to the learning needs of your child within SMS activities and events.

Limits to Confidentiality 

Confidentiality may need to be broken where required by law or where there is a serious and immediate safety concern, including where:

1. There is concern that the young person is in imminent danger of harming themselves.
2. There is reasonable belief that young person may seriously harm another person.
3. There is reasonable belief that a child or vulnerable person is being abused or neglected.

I have read and understood the information in this Consent form and have discussed anything I do not understand with the Priest. I am over the age of 18/ or the primary carer for the person joining SMS.
